The Southington High School athletic department has announced the first practices for the fall sports teams along with suggestions for incoming freshmen and returning student-athletes. In order for a student to compete in sports at Southington High School, athletes must submit a Sports Participation Permission form or parent permission form, the Concussion Education Plan & Consent form to be signed by both the parent and the student-athlete, the Sudden Cardiac Arrest Education Plan consent form to be signed by both the parent and the athlete, and the State of Connecticut Department of Education Health Assessment Record to be completed by the athlete’s physician. The health assessment record is only valid for 13 months. If the physical expires during the participating season, a new physical is required in order to continue participating at practices or games. Forms are available on the Southington High School website or the nurse’s office at the high school or middle schools. After the nurse receives the proper forms, she will present a yellow card to the student.
