Southington Care Center Executive Director William Kowalewski recently announced the hiring of Susan Urillo, RN, BSN, as the first director of transitions at the 130-bed skilled nursing and short-term rehabilitation community located at 45 Meriden Ave. Urillo coordinates discharge services and resources essential to the seamless transition of short-term rehab patients to home or other setting. She works closely with the Connecticut Center for Healthy Aging to ensure a positive outcome. “The transitions department starts working with short-term residents from the day of admission to identify potential needs upon the resident’s return to home. Our team of two social workers and two nurses are here as intermediaries to assist with the arrangement of services from long-term care, assisted living and short-term home care needs,” Urillo said in a press release from Southington Care Center. “We also identify potential barriers to discharge and work with the patient and family to provide the safest possible transition to home or other care needed.” This new position, coordinated with the Connecticut Center for Healthy Aging, is supported by the $2.1 million grant made last year by the Department of Social Services to Central Connecticut Senior Health Services, now Hartford HealthCare Senior Services.
